Add in "Best Conformation Rating" for dogs who have competed say in UDC but have not finished... change "ZTP" to "Breed Survey", it's more inclusive.

I really like this format!

Or, have you tried Google Forms? Sort of like an Access database but without requiring much knowledge?

BTW, this form was not really used, I just played with it. I just restored it from Google trash so I could see how it ended up in a spreadsheet, and here it is (info NOT current):

Another choice would be to just make a list and link to the dog's information on Dobequest... this is an open and easily user updated database.
